Application for New Jersey Dog License

**All Dogs over 7 months of age are required to be licensed annually in New Jersey**
Current proof of Rabies inoculations must be provided (attach copy) at time of registration with an expiration date
valid through November 1st of the current year pursuant to N.J.S.C. 8:23A-4.2.

Initial proof of spaying or neutering must be provided to qualify for the reduced license fee.
